Graduate Degree Re-Evaluation Request Instructions

  1. Download the Graduate Degree Re-Evaluation Request form
  2. Complete the applicable fields, print and sign the form (penned signature is required)
  3. Scan your completed form and upload it below by using the ‘Browse’ feature.
  4. A signed/completed request form and payment are required. Financial obligations to the University must be satisfied before the degree re-evaluation is processed.
We only accept single files which are 25mb or less each and can only be of the types
(JPG, TIFF, PNG) and Document files (CSV, XLS, XLSX, DOC, DOCX, PDF, TXT).
