A signed/completed request form and payment are required. Financial obligations to the University must be satisfied before the degree re-evaluation is processed.
We only accept single files which are 25mb or less each and can only be of the types
(JPG, TIFF, PNG) and Document files (CSV, XLS, XLSX, DOC, DOCX, PDF, TXT).
